You are kindly requested to complete this form and send it with the returning of the defective product. At the shipment we need a clear reference of the service request number. The helpdesk is entitled to return products without a proper RMA/ DOA form or without clear reference of the service request number. For products that prove not to be defect the helpdesk will charge an inspection fee.
